Action Quake is a fantastic mod, whether you choose to go at it you versus the rest of the online world or my favorite, TeamPlay! Choose from a regular weapon (Sawed-off Shotgun, M-4, throwing knives) and a specialty item (Kevlar vest, stealth slippers, bandolier, grenades).
Then: Lights, Camera, Action! and you're off, wielding your weapon and trying not to get shot in the head. Get injured and you have to bandage, limping off behind a bush to avoid getting shot while helpless. Shoot up the other team and you're up a point and go into the next round.
It takes some getting used to. I just about threw myself a party when I got my first frag other than myself. But this is one of my favorite mods as of late, including Freeze Tag (wickedwicked) and LMCTF.
There is of course the issue of LPB (Low Ping B*stards) who will always have the advantage, but if you're skilled enough you'll learn to cope. Also try to hit servers where Z-bots are banned.
Good servers to hit: El Gato de la Muerte and Ned's Action Teamplay
I also suggest for the avid Quaker to get yourself a registered copy of GameSpy. Twenty bucks for a lifetime helper/server finder with Quake and Unreal, amongst others.
This mod is a lot more realistic than the original Quake and so if you're a parent it might be a good idea to use your good judgement. I read in PC Gamer that a boy in Argentina, I believe, went into a movie theater pretending he was Duke Nukem and shot it all up. The kicker was that he shot up a mirror, because in the game there are goodies behind the mirrors. EEEE!!!! I like a stress-relieving blood-and-gore game as much as the next person, but use good judgement, please.
