New Page 2 New Page 1 Acetazolamide 250mg, 100 Tablets Product Description: Used primarily to treat hyperkalemic periodic paralysis (HYPP), a genetic disease of the neuromuscular system. Also used to decrease intraocular pressure in the treatment of glaucoma in animals. Drug Class: Diuretic Adverse Effects: Acetazolamide can potentially produce hypkalemia in some patients. Contraindications , Precautions, and Drug Interactions: Do not use in patients with acidemia. Use with caution in any animals sensitive to sulfonamides. Small Animal Dosage Information: Dogs and Cats: For glaucoma: 5-10 mg/kg, Every 8-12 hours, by mouth. For other dosage information see your veterinarian. *When used to treat glaucoma, monitor patient's ocular pressure. New Page 1 What is HYPP? Hyperkalemic Periodic Paralysis Disease (HYPP) is a muscular disease that affects both horses and humans. It is caused by a hereditary genetic defect that disrupts a protein called a sodium ion channel, a tiny gateway in the membrane of muscle cells. The genetic defect disrupts the channel's normal opening and closing, such that uncontrolled sodium influxes occur. These influxes in turn change the voltage current of muscle cells, causing uncontrolled muscle twitching or profound muscle weakness. High levels of potassium in the blood usually are present when the disruptions in the ion channel occur. What Are Effects of HYPP? Horses with HYPP can experience unpredictable attacks of paralysis which, in severe cases, can lead to collapse and sudden death. The cause of death usually is cardiac arrest and /or respiratory failure. The disease is characterized by intermittent episodes of muscle tremors manifested by generalized or localized shaking, trembling and weakness. Occasionally, episodes are accompanied by respiratory noises resulting from paralysis of the muscles of the upper airway (larynx and pharynx). In cases of mild attacks, muscle tremors may be so subtle as to be detectable only by an experienced clinician performing EMG testing. Can Symptoms of HYPP Vary in Severity? Clinical signs of HYPP do vary widely among different horses. Homozygous horses are affected more severely than heterozygous horses. Under ideal management practices, the defective gene does not appear to have adverse effects, but stress and/or increased potassium in the serum can trigger clinical signs of muscle dysfunction. Why some horses manifest severe signs of the disease and other exhibit little or no signs is unknown but currently under investigation. Unfortunately, a horse carrying the defective gene but showing minimal signs has the same chance of passing the gene to future generations as does the affected horse with severe signs. Lysodren Miotane Prescriber Highlights - Adrenal cytoxic agent used for medical treatment of pituitary- dependent hy- peradrencorticism Caution: pregnancy, diabetes, and preexisting renal or hepatic disease Adverse Effects: Lethargy, ataxia, wekness, anorexia, vomiting, and/or diarrhea; liver changes possible. Relapses are not uncommon All dogs receiving mitotane therapy should receive additional glucocorticoid supplementation if undergoing a stress (e.g., surgery, trauma, acute illness) Monitoring is mandatorty Avoid human exposure Drug interactions Chemistry Mitotane, also commonly known in veterinary medicine as DDD, is structuarally related to the infamous insecticide, chlorophenothane (DDT). It occurs as a white, crystalline powder with slightly aromatic odor. It is practically insoluble in water and soluble in alcohol. Pharmacology While mitotane is considered an adrenal cytotoxic agent, it apparently can also inhibit adrenocortical function without causing cell destruction. the exact mechan- nisms of action for these are not clearly understood. In dogs with pituitary- dependent hyperadrenocortic ism (PDH), mitotane has been dem- onstrated to cause severe, progressive necrosis of the zona fasciculata and zona reticularis . These effects occur quite rapidly (usually within 5-10 days of starting therapy). It has been stated that mitotane spares the zona glomerulosa and therefore aldosterone synthesis is un- affected. This is only partially true, as the zona glomerulosa may also be affected by mi- totane therapy, but it is uncommon for clinically significant effects on aldosterone produc- tion to be noted with therapy. Uses/Indications Inveterinary medicine, mitotane is used primarily for the medical treatment of pituitary- dependent hyperadrenocortic ism (PDH), principally in the dog. It has also been used for the palliative treatment of adrenal carcinorma in humans and dogs. Contraindications /Precautions Mitotane is contraindicated in patients known to be hy- persensitive to it. Patients with concurrent diabetes mellitus may have rapidly changing insulin requirements during the initial treatment period. these animals should be closely monitored until they are clinically stable. Dogs with preexisting renal or hepatic disease should receive the drug with caution and with more intense monitoring. some clinicians recommend giving prednisolone at 0.2 mg/kg/day during the initial treatment period (0.4 mg/kg/day to diabetic dogs) to reduce the potential for side effects from acute endogenous steroid withdrawal. Theophylline 100 mg, 100 Tablets Theophylline is a bronchodilator used in the treatment of heart failure, pulmonary edema, bronchial asthma and chronic obstructive pulmonary disease. Uses Theophylline is a bronchodilator used in the treatment of heart failure, pulmonary edema, bronchial asthma and chronic obstructive pulmonary disease (COPD). Availability Theophylline is available as tablet, long-acting tablet or capsule and liquid. How this medicine should be used Theophylline is not FDA approved for use in veterinary medicine; however, it is a commonly accepted practice to use this medication in dogs and cats. The usual dose of theophylline in dogs is 2.25-5mg/pound every 8 hours. The usual dose in cats is 2mg/pound every 8-12 hours. When the long acting dosage forms are given to dogs, the dose is 10mg/pound every 12 hours. Give theophylline on an empty stomach, 1 hour before or 2 hours after a meal. If using the long acting form, do not chew, crush or allow to dissolve on food. Side effects Side effects when using theophylline include vomiting, central nervous system excitement, fever, rash and palpitations. Side effects usually subside with dosage adjustments and/or continued therapy. Special precautions Theophylline should not be used in patients allergic to this class of medication. Do not use in pregnant or lactating animals. Theophylline should be used with caution in patients with severe heart disease, liver or kidney disease, hyperthyroidism, hypertension or stomach ulcers. Certain medications such as phenobarbital and phenytoin may decrease the effect of theophylline. Certain medications such as cimetidine, erythromycin, levothyroxine and quinolone antibiotics such as Baytril may cause an increase in the effect of theophylline. Always tell your veterinarian and pharmacist what other medications your pet is taking. In the event of an overdose Contact your veterinarian or veterinary emergency room. Methylprednisolon e 4 mg, 100 Tablets Anti- inflammatory medication indicated for a number of dermal, allergic, ocular, otic and musculoskeletal conditions. Methylprednisolon e 4mg, 100 Tablets Product Description: The indications for Methylprednisolon e Tablets are the same as those for other anti- inflammatory steroids and comprise the various collagen, dermal, allergic, ocular, Otic, and musculoskeletal conditions known to be responsive to the anti- inflammatory corticosteroids. Representative of the conditions in which the use of steroid therapy and the benefits to be derived there from have had repeated confirmation in the veterinary literature are: (1) dermal conditions, such as non-specific eczema, summer dermatitis, and burns; (2) allergic manifestations, such as acute urticaria, allergic dermatitis, drug and serum reactions, bronchial asthma, and pollen sensitivities; (3) ocular conditions, such as iritis, iridocyclitis, secondary glaucoma, uveitis, and chorioretinitis; (4) Otic conditions, such as otitis externa; (5) musculoskeletal conditions, such as myositis, rheumatoid arthritis, osteoarthritis, and bursitis; (6) various chronic or recurrent diseases of unknown etiology such as ulcerative colitis and nephrosis. In acute adrenal insufficiency, Methylprednisolon e may be effective because of its ability to correct the defect in carbohydrate metabolism and relieve the impaired diuretic response to water characteristic of primary or secondary adrenal insufficiency. However, because this agent lacks significant mineral corticoid activity, the parent hormones, Solu-Cortef containing hydrocortisone sodium succinate, Cortef containing hydrocortisone, or cortisone should be used when salt retention is indicated. Pharmacology: Methylprednisolon e, a potent anti- inflammatory steroid synthesized and developed in the Research Laboratories of The Upjohn Company is the 6-methyl derivative of prednisolone. It has a greater anti- inflammatory potency than prednisolone and even less tendency than prednisolone to induce sodium and water retention. Its advantage over the older corticoids lies in its ability to achieve equal anti- inflammatory effect with lower dose, while at the same time enhancing the split between anti- inflammatory and mineral corticoid activities. Dosage and Administration: For oral use in dogs and cats. Dosage: Average total daily oral doses for dogs and cats are as follows: 5 to 15 lb body wt 2 mg 15 to 40 lb body wt 2 to 4 mg 40 to 80 lb body wt 4 to 8 mg The total daily dose should be given in divided doses, 6 to 10 hours apart. Isoxsuprine causes these arteries to dilate. INDICATIONS Isoxsuprine has been used in the treatment of navicular disease and laminitis, where it is presumed that the disease is associated with some degree of decreased blood supply. Isoxsuprine also has been used to relax the uterus during the treatment of uterine infections. In other species, it has been suggested that isoxsuprine can be used to attempt to treat premature labor. DOSAGE AND ROUTE Of ADMINISTRATION Isoxsuprine is given orally in a dose ranging from 0.2 mg/kg to 0.6 mg/kg, depending on the treating veterinarian's discretion. This dose is given 2-4 times daily. SIDE EFFECTS/TOXICITY The major side effects concern the cardiovascular system. Isoxsuprine causes the systolic blood pressure (e.g., 150/100, the upper number, 150 being systolic) to rise, while the diastolic pressure falls. Reported side effects in other species include nervousness, trembling, weakness, dizziness, palpitations, nausea, and vomiting. If the horse appears to be developing side effects, consult the veterinarian regarding advisability of stopping this medication, or changing dosage. DRUG INTERACTIONS If a horse is receiving isoxsuprine therapy, the veterinarian must know this before treatment occurs with any anesthetic, tranquilizer, or narcotic drug. Cimetidine 800 mg, 100 Tablets Cimetidine is used in the treatment and prevention of stomach and intestinal ulcers in dogs, cats, horses and other animals. Cimetidine | Cimetidine HCl Cimetidine has been used for the treatment and/or prophylaxis of gastric, abomasal and duodenal ulcers, uremic gastritis, stress-related or drug-induced erosive gastritis, esophagitis, duodenal gastric reflux and esophageal reflux. It has also been employed to treat hypersecretory conditions associated with gastrinomas and systemic mastocytosis. Cimetidine has also been used investigationally as a immunomodulating agent (see doses) in dogs. Prototype histamine-2 blocker used to reduce GI acid production Contraindications : hypersensitivity. Caution: geriatric patients, hepatic or renal insufficiency Storage/Stability /Compatibility Cimetidine products should be stored protected from light and kept at room temperature. Do not refrigerate the injectable product as precipitation may occur. Oral dosage forms should be stored in tight containers. The cimetidine injectable product is compatible with the commonly used IV infusions solutions, including amino acid (TPN) solutions, but should be used within 48 hours of dilution. Compatibility is dependent upon factors such as pH, concentration, temperature and diluents used. Pharmacology At the H 2 receptors of the parietal cells, cimetidine competitively inhibits histamine, thereby reducing gastric acid output both during basal conditions and when stimulated by food, pentagastrin, histamine or insulin. Gastric emptying time, pancreatic or biliary secretion, and lower esophageal pressures are not altered by cimetidine. By decreasing the amount of gastric juice produced, cimetidine also decreases the amount of pepsin secreted.
